STATEMENT OF PURPOSE AND GOALS
The purpose of NORTH COAST SUPPORTABILITY, LLC is to increase the availability of opportunities for people with disabilities to live good lives in a place of their choice, to enjoy a lifestyle that is self-directed, to the greatest extent possible, and to interact with their community in ways that enhance and strengthen both.
SUPPORTABILITY, a Community Integration Service is designed to provide Community Integration services which blend the ideas of self-directed services, the principles of person-centered planning, the principle of supported living, personal empowerment and normalization into a comprehensive set of services aimed at enhancing the lifestyle of those served.

SUPPORTABILITY is designed to provide quality Community Integration Services for those with developmental disabilities that require significant supports and assistance in caring for themselves in the community, maintaining appropriate social behaviors and including “medically fragile” consumers that may not have the ability (including stamina) to participate in group-based activities. The services are designed to allow consumers to have a presence in their community. The goal of these services is to develop an understanding of the preferences and interests of the consumer. This information is then utilized to assist the consumer with developing an appropriate Service Plan. Each consumer receives an individually personalized set of objectives, curriculum, and schedule that best support the goals of the individual consumer.
These services are designed to enhance lifestyles of those served through the:
- Self-directed services provided through identification of individual consumer interests and preferences. This is done thru the individual assessment process and consumer directed scheduling.
- Identify and facilitate access to events, activities, and experiences in our community and work and volunteer related activities that relate to individual consumer interests and preferences.
- Identification of and facilitated inclusion into preferred activities.
- Creative exploration into translating interests into new potential activities, including potential work and volunteer opportunities.
- Training and supports needed to meet the community inclusion goals of individual consumers. This includes mobility training, work exploration and public transit access.
- Identification and documentation of individual consumer’ communication abilities, techniques and style with special emphasis on how they use their behavior to communicate to others.
- Development of curriculum aids that facilitate communication goals, including sign language, PEC system, etc.
- Close collaborative efforts through communication and coordination with home support service providers, parents and or other support providers.
Individuals with significant disabilities and severe reputations have often not had the opportunity, and thus are not familiar with the process making choices and being empowered in their daily lives. SUPPORTABILITY offers an opportunity to participate in a supportive process of learning, with the consumer, to develop a working understanding of the consumers’ interests and preferences. This will then become the basis for direction of specific curriculum content, schedules and experiences.
We believe that Community Integration Service providers should see consumers as individuals. Consumers deserve to be served in a manner that reflects their personal identity, history, culture and lifestyle. All factors of their character and contribution should be taken into account when designing appropriate services. How people relate to and interact with their community is only one aspect of their overall lifestyle. Choice and personal satisfaction of the consumer should drive service and curriculum decisions.
The purpose of SUPPORTABILITY is to provide Community Integration Services. These contracted services are community based, on a one-to-one staffing ratio standard. All services are provided in community settings, the consumer’s home or other environments as necessary based upon the self-directed schedules developed for each consumer. All services will be conducted by Community Instructors, under the supervision of a qualified Field Coordinator. In addition, to comply with Abx9, Supportability offers services to provider volunteer and work-experience for consumers that seek this option. These services include development of a “Positive Personal Profile,” job search, skill development and job training.
